Choose the request form on your website

Pick which of your request forms your website's contact page shows. Leads from it land in Requests, marked as from your website.

Who can do this
People with the Website permission

Your website's contact page shows one of your request forms. Every lead from it lands in Requests, and the list marks it "from your MakeHay website" so you can tell it from other leads.

  1. Step 1: Go to Settings › Website and pick Settings under Site.
  2. Step 2: Under Request form on the contact page, pick one of your forms, or The first active form to use your oldest live form.
  3. Step 3: A message says the form is saved. The live site shows it right away, with no need to publish.

Only live forms are in the list. If the form you picked is turned off later, the site uses your oldest live form instead.

Good to know: If the panel says no request form is active, go to Settings › Request forms, open a form's details and turn on Form is live. While the contact page is set to show a form and none is live, you can't publish changes.

Show or hide the form

  1. Step 1: Pick Contact under Pages and open the Contact section.
  2. Step 2: Turn Request form on or off. The same section has switches for Phone and text, Email, Address and directions and Hours.
  3. Step 3: Click Publish changes.

Tip: To change the questions the form asks, edit the form itself. See Take requests with a request form.
